August 2-8, 2026, is National Community Health Center week. Community Health Centers (CHCs) play a critical role in addressing acute and chronic conditions through prevention, early detection, and treatment. August 5th highlights efforts like vaccinations, cancer screenings, heart health, diabetes care, and managing chronic infectious diseases. Health Services of North Texas (HSNT) is proud to be one of hundreds of CHCs across the country helping Americans live their healthiest lives by addressing chronic and acute health issues through prevention efforts, screenings, and care.
Prevention
CHCs help their patients prevent diseases before they become serious. At your local CHC, disease prevention efforts can include:
- Yearly wellness visits
- Vacunas
- Health education
At each patient’s yearly wellness visit, their healthcare provider can help identify concerning symptoms and determine how to best address them. Vaccinations are also an important part of preventive care, as they can help keep patients from getting sick or lessen the effects of illness.
Healthcare providers and other CHC team members provide patients with health education on a variety of topics, including disease prevention. This education is tailored to each patient’s lifestyle and unique needs. CHCs are also equipped to connect patients with community resources that support a healthy lifestyle, such as food and housing assistance.
Screening and Early Detection
CHCs help prevent and treat conditions such as cancer, diabetes, and infectious diseases by offering screenings to detect these conditions for those who are at risk. For example, at age 45 it is recommended that everyone start being regularly screened for colorectal cancer (CRC). CRC is an extremely treatable condition if caught early but can be devastating to a person’s health if left untreated. Screening can give CHC patients peace of mind and help prevent conditions like CRC from turning into a health emergency.
In 2025, HSNT screened 54% of eligible patients for breast cancer, and 65% for cervical cancer. Our healthcare providers also screened 92% of eligible patients for depression.
Treatment
For many Americans living with chronic illness, CHCs are a crucial part of their treatment. At HSNT, we have a special focus on diabetes and hypertension, and our patient numbers show our providers’ dedication to helping patients with these conditions thrive in their daily lives. In 2025, 69% of HSNT patients with hypertension demonstrated control over their condition through their blood pressure readings. In addition, 71% of diabetic patients showed control through their A1c levels. These metrics were achieved through the hard work and dedication of patients and team members collaborating to improve patient health.
